Role Overview

  • As a Nursing Care Coordinator, you will be responsible for coordinating care for patients, especially those with complex or long-term conditions, managing care schedules, liaising with clients and carers, and ensuring the highest standards of care are maintained.
  • You will play a vital role in ensuring our clients receive timely and effective support while maintaining compliance with regulatory requirements.

Key Responsibilities

  • Coordinate and navigate care across the health and social care system, connecting patients with the right teams and services at the right time to enable the delivery of person-centred care.
  • Conduct comprehensive risk and needs assessments and develop personalized care and support plans based on what matters to patients.
  • Ensure efficient allocation of care staff to meet patient needs and manage care schedules for clients using digital care management system.
  • Work with GPs, other healthcare professionals, and community services to ensure a coordinated approach to patient care.
  • Maintain health and social records of clients, review care notes, and ensure care plans and risk assessments are updated as needs change using digital care management system.

Requirements

  • Bachelor of Nursing qualification.
  • Previous experience in a care coordination or adult nursing role.
  • Strong organisational and communication skills.
  • Ability to work under pressure and manage multiple tasks.
  • Proficiency in scheduling software and Microsoft Office.
  • A caring and compassionate approach to work.
  • Ability to work collaboratively within a team.
  • Commitment to maintaining confidentiality and exercising discretion.

Job Offer:

  • An estimated salary range of N150,000 – N200,000 per month.
